There's public healthcare but it's kind of a complicated system with different rules for the employed and unemployed, of course this only applies to mainland China, if you live in Hong Kong for instance you're on your own.
You have to pay for anything but the bare minimum treatment, as best shown by that one guy ranting about Chinese cheapness where a guy was run over by a car, brought to a hospital (since none of the actual Chinese would bother helping him) and was set up for treatment to check for potential broken bones and brain damage but once they found out the guy had no cash on him they just patched up the obvious bleeding parts and sent him away completely losing interest once they realized they'd get no cash out of him.
