For human moderation, that's fiscally impossible to deal with, even using cheap third-world labor. Indians cost around $2/hr, South America costs around $5/hr. Amerimutts and Chinese cost $10/hr.
And those are usually bottom-barrel rates, like you're not getting trained labor at that price, you're getting easily replacable monkeys that press yes/no based on a short list of very explicit rules.
So you put in automation. PhotoDNA is unironically one of the most successful apps in existence and can do some freaky-levels of recognition, but it only works on known CSAM.
