When it comes to the Inquisition, I read that the usual protocol for interrogations consisted mainly on the executioner simply showing the torture devices to the convict and explaining to them how they worked. This was usually enough to obtain the so-called "confession". So probably many torture devices actually existed but weren't used as regularly as we think.
The Spanish Inquisition's processes are actually fascinating to read about and are often much more humane than many other administrations. For example, they were only allowed to use tortures to confirm details after convictions, confessions obtained during torture were invalid and legally could not be used to convict somebody.
Tortures were graded in severity and the severity of a torture had to match the severity of crime that was being investigated. Not only that, but torture sessions were mandated to be in 15 minute increments, and there were only so many increments that could be applied based on crime severity, so someone accused of being a thief could only be tortured twice total over the course of their trial, where as a murderer could be tortured up to 8 times.
The Inquisition actually complained many times that regular criminals would blaspheme in the presence of the Calicifadors just so their crime would be religious in nature and transferred to the Inquisition, since they had so many regulations compared to the secular authorities.

Torture in the sense of inflicting the maximum possible punishment, in both the pain and spectacle sense, was real.
Torture in the sense of extracting information or confession was also generally real, but not necessarily done by the people typically accused of it.
The specialty torture tools were typically bullshit. You don't actually need much more than a table and a set of butcher tools to torture people.
