Mainly Irish and Germans, depending on where else you are in the country Dutch (Not Pennsylvania Dutch, actual Dutch), Scandis, and Czechs. There used to be a sizable rural black culture in the US but it's been significantly reduced, it's still around but not really a major aspect of it anymore.

Hillbillies in the Appalachians have always been Scotch-Irish overwhelmingly but you'll find in the Appalachians and Ozarks plenty of Anglo, German, French, etc surnames as well. These people have intermarried and been anglicized for a long time. Their communities in the hill country are dying slowly. No one speaks paw-paw French and other unique dialects anymore. All mutts.

In New England and Louisiana, the French Canadians contributed to hillbilly culture greatly. In northern NE a French last name is synonymous with being a dirt poor outdoorsy family. In western new Hampshire there are a great deal of towns full of Finns who are incredibly inbred and speak really dumbed down Finnish with tons of English mixed in-between. They also discriminate greatly against non-finns.
