The difference is that the discomfort in their own skin isn't an imposed one derived from societal standards imposed on them. But an internal feeling that's kind of innate. Why shouldn't one be allowed to change their form to make themselves more comfortable? In the case of a dysmorphic person their view of themselves is distorted so changing it doesn't help. But a dysphoric person does have an accurate view of their body and that accurate view causes dysphoria. I see it no different then someone who's fat being unhappy with their body and wanting to lose weight.
There are definitely people with actually dysphoria and have been since time immemorial, there’s a greater if not equal to number of people induced into dysphoria through exposure.
